I use SetNine Shoutcast Hosting http://shoutcast.setnine.com and am very happy with them. The server is always up, they are very helpful with tech support and they are cheap. They also seem to offer a bit more than the other hosts, including an RSS feed for my stream and a webpage for it as well.
